If we are not able to provide “in-network provider” coverage, we are happy to provide you with a superbill. We encourage every patient to call their specific insurance company as coverage and benefits can vary from plan to plan for every insurance and patient. For insurance verification questions, please call your health insurance company directly. Here’s a helpful verification form so when you call, you know what to ask.
Insurance Billing
Please bring your completed insurance verification form to your first visit. While we are happy to bill your insurance company, your contract is strictly between you and your insurer – Seattle Acupuncture Wellness is not a party to that agreement.
Coverage & Responsibility
You are responsible for all charges not covered by your insurance. Be aware that your insurer may or may not cover your initial consultation, follow-up, or additional examinations.
Payment & Claims
Any balance not paid by your insurance company must be paid directly to Seattle Acupuncture Wellness. We do not engage in disputes or appeals with insurers regarding denied or unpaid claims.

The Seattle Acupuncture Wellness Center is conveniently located in the heart of Northgate in Seattle, across the street from the Kraken Community Iceplex, just one door north of the Seattle Public Library Northgate branch, on 5th Avenue NE. Our main office and medical office building is wheelchair accessible.

The Seattle Acupuncture Wellness Center offers a cash (Non insurance) package with discounted rates if payment is made in advance.
We accept payment by cash, check, HSA (Health savings account)/FHA (Flexible savings account),credit/debit card or Venmo.
